			<content:encoded><![CDATA[<p><img src="http://rajukumar.files.wordpress.com/2008/03/45_04.thumbnail.gif" alt="45_04.gif" />We often consider making paper to be a very complicated, skilled process and of course, that can be true. But it&#8217;s not hard to craft paper yourself at home with very little equipment. The only special item you&#8217;ll need is a mould. This is a wooden frame with a mesh stretched across it to act as a filter or sieve. It&#8217;s very simple to make your own mould &#8211; just fasten four pieces of wood together in a rectangular shape to make the frame. Use 3/4&#8243; x 2&#8243; wood (20 x 50 mm) and make the inner dimensions of the frame the size of the paper you want to make. The frame needs to be about 2&#8243; (200 mm) in depth. Then cover the frame with tightly stretched mesh &#8211; mosquito net or just a piece of net curtain is fine. Make sure it&#8217;s securely fastened to the edge of the frame. You&#8217;ll also need a large container to hold water, about the size of a baby bath.</p>
<p>Now you&#8217;re ready &#8211; let&#8217;s get started! To make the pulp collect together a variety of unwanted paper (this craft is great for recycling). avoid newspaper (not pretty) and glossy magazines (the gloss makes them unsuitable) but you can use tissues, art paper, envelopes, discarded letter paper or copier paper &#8211; these are all suitable. Then tear the paper into about 1&#8243; (25 mm) square pieces and soak it in water. The soaking time depends on the paper you&#8217;re using. Tissues will take under an hour, while high quality paper may take up to 3 days. Using hot water will speed up the process.</p>
<p>The next step is to macerate the paper in a blender or food processor. Add a couple of cups of water to each small handful of the paper. (225 ml). You only want to blend until the fibres separate, so be cautious and use short bursts. If you blend for too long your paper will be very smooth and you won&#8217;t have that rustic, handmade quality which is so attractive. Put the blended pulp into another container. A blenderful will make about 1 letter-sized sheet, so continue until you&#8217;ve blended as much as you think you&#8217;ll need.</p>
<p>Now comes the even more fun part &#8211; making the paper! You can add all sorts of small things to give your paper an individual feel. For a country theme add some tiny feathers or pressed leaves and flowers. For Christmas or a child&#8217;s party you can add sparkles, or tiny foil shapes. If you&#8217;d like to colour the paper, you can use natural dyes like blueberry or beetroot juice, onion skins, turmeric or coffee grounds, or you can purchase commercial dyes from craft stores. Just stir your dye into the pulp, adding a little at a time until you have the colour you want. <p align="left">Protein in plants and our bodies is based on nitrogen; nitrogen is the building block of life. More nitrogen in the soil allows farmers to grow more crops. However, the soil becomes poorer in minerals unless farmers replace them.</p>
<p>Minerals are simple inorganic chemicals required by living organisms. They act as catalysts for vital processes. Plants draw their mineral salts and trace minerals from the soil while animals ingest theirs from their food. Magnesium is used by plants for photosynthesis while humans use it, together with zinc, for the production of hormones and maintaining strength levels. There are many trace minerals which we know are important to human nutrition but do not know exactly how.</p>
<p>Minerals came to my attention many years ago when I received an audio cassette in the mail from somebody network-marketing supplements. I had tried taking vitamins but had not noticed any difference except my urine turned a different color, so had dismissed the idea of nutritional supplementation.</p>
<p>The cassette contained the famous lecture &#8216;Dead Doctors Don&#8217;t Lie&#8217; by Joel Wallach. Its premise is that we all suffer from a deficiency of minerals and that the medical profession profits from our malnutrition. This is an extreme view of the medical profession; that it has an interest in the population remaining malnourished. You do not have to subscribe to this viewpoint to agree that our diet is deficient in minerals. It is deficient because our food production is industrialized. Michael Pollan, in his excellent book &#8216;A History Of Four Meals&#8217;, gives us a short history that helps us understand why minerals lost out in the effort to harvest nitrogen.</p>
<p>Before the invention of nitrogen-fixing, the supply of nitrogen on Earth was severely limited even though 80% of the Earth&#8217;s atmosphere is nitrogen. Until then, the only way to &#8216;fix&#8217; nitrogen, to capture it and put it in the soil so it could feed plants and us, was to plant legumes in the fields. The amount of grains that farmers could grow was severely limited by the amount of nitrogen available in the soil. Farmers had to rotate crops so that corn could only be grown every other year, interspersed with years of growing legumes to put back nitrogen and other nutrients. They would also graze farm animals and spread manure from livestock in the fields to put back more nutrients.</p>
<p>In 1909, a German chemist called Fritz Haber invented a process to fix nitrogen from the atmosphere and convert it into ammonia which could then be used to make ammonium nitrate. He drove this technical development in order to manufacture nitrates for explosives in support of the German side in the First World War. He was also responsible for the development of such poison gases as ammonia, chlorine and Zyklon B which was used in Hitler&#8217;s concentration camps. Only later did his invention find a use in agriculture.</p>
<p>Michael Pollan identifies 1947 as the year when the era of the industrialization of food production began. A munitions factory at Muscle Shoals, Alabama which used the Haber-Bosch (Bosch commercialized the process) nitrogen-fixing process had a huge surplus of ammonium nitrate, the main ingredient in making explosives. It switched over to manufacturing chemical fertilizer in order to use up this surplus. The Department of Agriculture decided to use the ammonium nitrate as fertilizer on farmland.</p>
<p>The old methods of crop rotation and depending on vegetables and grazing animals to restore nitrogen and other nutrients to the soil became unnecessary once farmers could purchase fertility in the form of bags of ammonium nitrate fertilizer.</p>
<p>If Haber had not found this way of artificially supplementing the nitrogen in farm soil, the population explosion associated with industrial expansion could not have happened. Pollan refers to a book by Vaclav Smil, Enriching The Earth, that estimates that two out of every five humans alive today would not be alive without Haber&#8217;s invention. He rates the invention of fixing nitrogen as the most important of the twentieth century.</p>
<p>The end of crop rotation and the constant, highly intensive cultivation of one or two crops (usually soybeans and corn) made possible by the use of ammonium nitrate fertilizer mean that minerals are taken out of the soil and, if put back, are only put back artificially and incompletely by farmers. Typically, apart from the nitrogen in ammonium nitrate, they only put back a few minerals, the main ones being potassium and phosphorus. They may also put calcium, sulfur, magnesium and sometimes boron, manganese, iron, zinc, copper and molybdenum. They do not replace the minerals that our bodies need.</p>
<p>Therefore, what we have gained in the availability of nitrogen and the quantity of food that we can grow, we have lost in nutritional value from essential minerals. We cannot rely on industrial food to provide the minerals we need.</p></div>

			<content:encoded><![CDATA[<div>A new industry of high-technology rehab and physical therapy billing has mushroomed under the auspices of its promise to streamline the collections process and leave doctors with more time to care for their patients. Though many high-quality services and systems exist, an overwhelming variety of options and attractive (yet unsubstantiated) performance claims from some providers have charmed busy doctors into making poor strategic decisions for their practices.</p>
<p>&#8220;No physical therapy or rehab education program prepares us for the harsh realities of daily struggle with insurance companies to get paid in full and on time,&#8221; says Yaffa Liebermann, PT, GCS, and CEO of Prime Rehabilitation Services, Inc. &#8220;A recent physical therapy program graduate lacks basic knowledge about payer-provider adversity, HIPAA compliance, audit mechanics, SOAP note management, reporting tools, billing profitability metrics, and outsourcing opportunities. And no program teaches us how to use modern computer technology and Internet to accumulate and leverage our collective health care and practice management knowledge.&#8221;</p>
<p><b>Billing Quality Statistics </b></p>
<p>In order to understand how bad the providers&#8217; financial situation is, it is important to recognize that in an average practice, 17.7% of accounts receivable are 120 days past due. In other words, about 1 in 5 procedures billed today won&#8217;t get paid until four months from now. Although this may not seem to be a problem, as it would be expected that the money will come eventually, in fact an unpaid claim that is 180 days overdue has less than 1% chance of ever being paid. This may be good news for insurance companies, but it is certainly bad news for physical therapists.</p>
<p>Importantly, the very fact that these statistics are news to some practices is part of the problem: many rehab or physical therapy practices don&#8217;t even know their basic financial parameters such as &#8220;AR past 120.&#8221;</p>
<p><b>Uneven Playing Field </b></p>
<p>Providers and payers are playing tug-of-war on opposites sides of the same claims, but the payers have made significant investments in infrastructure and personnel. Providers, on the other hand, are playing in the worst of conditions. Except for the initial claim submission, they are completely passive at every step. They wait for the payer to review the claim, wait to receive the errors, wait for the review of the corrected submission, and wait, and wait.</p>
<p>In the physical therapy practice this &#8220;waiting&#8221; can be difficult to appreciate because everyone is busy with new submissions, resubmissions, and appeals. There is little time to take a more active role.</p>
<p><b>Vericle &#8211; a Shared Repository of Billing Solutions </b></p>
<p>The rules of the game will not change in the foreseeable future, and the payers will continue to own the tables for at least that long, but today&#8217;s physical therapy practice is not doomed to lose every hand. More and more practices are learning that by playing the game smarter, they can spend less time thinking about collections and more time with patients. What&#8217;s the secret? It boils down to finding errors before the payer even knows the claim exists, to formulating and encoding the rules about finding and correcting the errors, and to insisting on a disciplined follow up on every delay and underpayment.</p></div>
